In order to use this system, each patient must have a email account that will be used to communicate between the patient and the clinic. Adult patients can easily sign up for FREE email accounts.
Children email accounts must be established by an adult parent or guardian.

Please note: Before you can access the Patient Portal you are required to sign a PIN authorization/release form which will allow a Sixteenth Street team member to print your personal PIN letter.
(PIN letters are given only to the patient face to face for security purposes)
For security purposes each patient must have a unique email account. For families with only one household email address it is recommended to open a free email account with Gmail, Hotmail , Live or Yahoo for your
other family members. You can always use your main email account to receive notifications; all you have to do is fill out the alternate email address box with your main account.
